Preparing for your visit

consultYou have been scheduled to see Mark Green, MD. To help make your visit as efficient and comfortable as possible, prior to your visit please download and complete the *Initial Consultation Form and the Registration Forms, which can be found on this web site. Bring these forms with you to your initial appointment.

If it is not possible to keep your appointment, please call at least one day prior to the scheduled date. Please arrive on-time for your appointment.

buildingOur office is located at 16 East 60th Street, on the southwest corner of the intersection of Madison Avenue and 60th Street. There is a silver-colored awning over the entrance. The office is on the third floor, suite # 310.

If you come by automobile, unless you are very lucky and find a parking place, you must park in a garage. The garages are expensive; the closer the garage to the office the more expensive it is. There are garages at the following locations:

  • East 60th Street between Madison and Fifth
  • East 61st Street between Madison and Fifth
  • East 60th Street between Park and Lexington
  • East 62nd Street between Lexington and 3rd Avenue

If you come by subway, the closest station is the N/R line at 5th Avenue. When you exit the station walk east on 60th Street from 5th Avenue. Columbia-Presbyterian Eastside is the last entrance on the right just before Madison Avenue.

Please be sure to obtain and bring with you any medical records (reports, history, x-rays) that have been done in the past.

The fee for your initial visit is $600.00, and must be paid at the time of the visit. We participate with only a few insurance programs for direct billing. If you have appropriate documentation for one of these insurance programs, including a referral if necessary, you will be expected to pay only your standard co-pay at the time of the visit. Patients eligible for Medicare should bring their Medicare card with them. If you are covered by an insurance program that we do not participate in, you are expected to pay the entire fee at the time of the appointment. Following your appointment, you will be provided with a standard insurance billing form, which can be used to obtain reimbursement from your insurance company. If laboratory tests are ordered, billing and payment for these is required at the testing site at the time of the testing. Our office and the hospital accept Master Card, Visa and American Express, in addition to cash or check.
